One (1) copy of School ID (long size) with three affixed signatures
Blue ink pen
👨👩👧👦 If claiming via parent/sibling, submit:
Two (2) copies of authorization letter
Two (2) copies of grantee’s School ID (with three affixed signatures)
Two (2) copies of authorized person’s valid ID
📌 All documents must be printed in long size.
