Qualified enrollees who will enroll for Graduate School 1st Semester A.Y. 2023 – 2024 should report at the Registrar’s Office for the submission of enrolment requirements not later than August 18, 2023. Below are the enrolment requirements:
Official Transcript of Records with mark “for enrolment purposes”
Honorable Dismissal issued by the school last attended (for transferee)
Form 212 – Personal Data Sheet
1 pc. white long-size folder
1 pc. 2×2 recent colored picture with white background
Marriage contract for married female
NOTE: All documents must be in original copy except Marriage contract
