To download, login your OPAS account (not email), click the CONFIRM & PROCEED button then answer and complete the surveys. Finally, click DOWNLOAD TEST PERMIT button and PRINT the pdf file in a LONG SIZE BOND PAPER.
ON THE DAY OF THE EXAMINATION (January 18, 2025), don’t forget to bring the following:
📌printed TEST PERMIT downloaded from the OPAS
📌Any VALID ID or proof of identification (copy of birth certificate FOR NO VALID ID)
📌Pencil, eraser, sharpener
📌Long size white folder
📌2”x2” ID picture in white background with name tag (Full handwritten LAST NAME, FIRST NAME M.I. and signature above the name) attached to the test permit
📌Photocopy of the Grade Credentials
📒 For Graduating SHS: SF9 (Report Card) or SF10 (Form 137) of your Grade 11
📒For SHS Graduate: SF9 (Report Card) or SF10 (Form 137) of your Grade 12
📒For Transferee: Official Transcript of Records or Certificate of Grades with general weighted average
